HELPFUL TIPS FOR INSURANCE:
Therapy services may be covered in full or in part by your health insurance or employee benefit plan. If choosing to use your health insurance, you are still responsible for all or a portion of the session. Please check your coverage carefully by asking the following questions:
Do I have mental health/substance abuse insurance benefits?
What is my deductible and has it been met? (This applies to both in network and out-of-network benefits.
How much is my copay per session? How many sessions per year does my policy cover?
What is the allowed amount per therapy session?
Is approval required from my primary care physician?
CANCELLATION POLICY
I understand that your time is valuable and hope you understand that of mine. I also understand that life happens! In the event that you are unable to to keep your scheduled appointment, please give me at least 24 hours notice. Cancellations with less than 24 hours notice will be charged a $65 cancellation fee. No shows with exception to extraneous circumstances will be charged the hourly fee.
CONSULTATION:
If you are looking for the right fit in a therapist, I encourage you to schedule a free 20-minute phone consultation. I am happy to address any questions or concerns, and to offer you a sense of my approach. In addition, we can briefly discuss what you are hoping to get from therapy.
INSURANCE COVERAGE:
At this time I do accept OHP. I do not accept other private insurance companies. However, your insurance may accept my services as “out-of-network”. This means that you will pay the full cost of session up front, and I will provide you with a formalized statement you can submit to your insurance company for reimbursement. For more information about how “out-of-network” billing works, please contact your insurance provider to see if your plan includes mental health benefits.
SHARING PERSONAL INFORMATION
I understand that there’s a common belief that therapy means revealing every detail and going through the whole story of “what happened.” However, my approach is different. I recognize that as a therapist, I’m initially a stranger to you, and trust is something that needs to be built over time. In our sessions, you are in control.
You choose to share as much or as little as you feel comfortable with.
We’ll go at a pace that feels right for you.
INITIAL SESSION:
Once you’ve set up your first appointment, I’ll email you the necessary documents. Please review them and bring or email them to me to review at our scheduled time. Our first session will be a time to review your paperwork, start to get to know one another, share and identify your goals for therapy.
*You will receive an informed consent form and an intake form. The informed consent form essentially serves as our “contract” to work together, so please take some time to review it thoroughly before signing. It’s important that you feel comfortable with the terms and conditions we’ll be working under. If you have any questions or concerns about these documents, please don’t hesitate to email me at any time, or you can bring them to our first session for us to discuss together.
PAYMENT
Payment is expected at the time of services unless other arrangements have been made. I accept cash or check
